Active Reading Form - Before, During, and After

Form that students complete during guided or independent reading focusing on reading skills to be used before, during, and after reading. Before skills include identifying genre, author's purpose, accessing prior knowledge, purpose for reading, predictions and questions. During skills include predictions and questions, and "stop and jot" section with prompts to write answers, interesting facts, and main idea. After skills include reaction/reflection statement with prompts, summary, and rating.
